Parental Absence Authorization Form

This form is for a parent/guardian who is unable to attend a co-op meeting, but their kids will be attending. Please print, complete, and return this form to designate the adult who is responsible for them.

NOTE: This is for kids in the kids building only! Please return this completed form to the help desk on or before the day of absence.

Parent Involvement Agreement

This form is for parents of kids participating in co-op at the kids building. Please print, complete, and return this form before the first day of co-op.

Liability Release Form

This form is for all parents that have kids participating in co-op. Please print, complete, and return this form before the first day of co-op.

Forms for Teachers

Teacher Commitment Statement

This form needs to be completed each year by all our teachers. Please print, complete, and return this form by August. 15.

Background Check Form

This form needs to be completed by all our new teachers and teachers whose background check needs to be renewed. The background check is good for 4 years. Please print, complete, and return this form by August. 15.
